Ingrown nails



Ingrown nails are nails whose corners or sides of the nails dig painfully into the skin, often causing infection. They are frequently caused by improper nail trimming but also by shoe pressure, injury, fungus infection, heredity, and poor foot structure. Toenails should be trimmed straight across, slightly longer than the end of the toe. If the ingrown portion of the nail is painful or infected, our podiatrist may remove the affected portion; if the condition reoccurs frequently, our podiatrist may remove the ingrown nail surgically under local anaesthetic.

外遊過後,靜脈栓塞?

接連的聖誕、新年長假期,港人紛紛外遊去,但歐遊、美加遊動軏要坐10多小時飛機,若本身已是患上靜脈曲張人士,在機上又未有好好做足準備,出現靜脈栓塞的風險往往大增,一旦出現深層靜脈栓塞,血塊被沖到肺部造成肺栓塞,後果更是可大可小。


經過剛過去的一段長假期後,因靜脈栓塞而求診的個案數字,的確有所上升。深層靜脈栓塞即俗稱的「經濟客艙症候群」,指長時間乘坐交通工具時,因雙腳垂下加上活動少,靜止的血液積聚在腿部靜脈,並型成血塊,若是血塊經靜脈被沖到肺部,可造成致命的肺栓塞。


臨床上,靜脈曲張患者,較易出現靜脈栓塞問題,淺層靜脈栓塞會令患處發炎、腫痛,而嚴重的深層靜脈栓塞,則可帶來即時的生命危險。為健康著想,靜脈 曲張患者在乘坐長途航班時,應穿上醫療用的壓力襪,並多進行舒展足部的活動;即使是一般人士,亦有機會因久坐而出現靜脈栓塞,建議大家在機上少喝酒、咖啡 等飲品,定時站起來活動一下,不要只沉迷在螢光幕前「煲劇」、看電影而忽略靜脈栓塞的致命風險!

Foot odor




Foot odor results from excessive perspiration from the more than 250,000 sweat glands in the foot. Daily hygiene is essential. Change your shoes daily to let each pair air out, and change your socks, perhaps even more frequently than daily. Foot powders and antiperspirants, and specialized shoe and socks or insoles can help lessen odor.

靜脈曲張炎 嚴重可致命

切勿輕視

每逢長假期後,都會有較多嚴重的靜脈曲張患者求診,當中部分更出現「淺表血栓性靜脈」(Superficial Thrombophlebitis) 。淺表血栓性靜脈炎即有血塊在表淺靜脈內積聚並發炎,是靜脈曲張其中一種併發症,多見於中期以後的中年患者。外科專科醫生謝卓華呼籲,切勿輕視此併發症的 嚴重性,一旦血栓蔓延至深層靜脈,會引致「深層靜脈栓塞」(Deep Vein Thrombosis),若血栓沿靜脈流至肺部,隨時會危害性命。

逾70歲的林婆婆育有3名子女,患有靜脈曲張多年,小腿常感到痠痛及血管腫脹,但一直未有理會,直至最近到俄羅斯旅行後,右腳近小腿位置有硬青筋凸起,腫脹情況更蔓延至大腿內側,令她舉步維艱,且感到很疼痛。

初時她以為是扭傷腳,但因復活節假期臨近,她待假期後才求診,怎知檢查時已確診患上急性淺表血栓性靜脈炎,於短短兩星期,右腿的血栓已由小腿蔓延至大腿的大隱靜脈,需進行大隱靜脈結紮手術,令血塊不再蔓延。

患處有硬塊影響步行


不少人會奇怪,為何小小的靜脈曲張會引致隨時致命的併發症,事實上,若靜脈曲張持續惡化,倒流的血液會在表淺靜脈內囤積形成血栓,出現急性非化膿性 炎症。初期患處會疼痛、紅腫及發熱,若觸摸患處,會感到有硬塊,影響步行。血栓更可能會蔓延至大隱靜脈,或進一步到達深層靜脈,引致下肢腫痛;若血栓經靜 脈流至肺部,更會引致肺部靜脈血管栓塞,出現呼吸困難及心跳加速等,最嚴重可致命。

 


長期坐交通工具易惡化

謝醫生指一般情況下,輕微的淺表血栓性靜脈炎,可服用消炎藥便可消退炎症,待炎症過後便應接受手術,根治靜脈曲張。否則,復發率是很高的,而嚴重個 案則需要盡快進行手術治療。但不少患者卻輕視靜脈曲張的嚴重性,很多時沒有盡早求醫,即使早有不適徵狀,也延至長假期後才求診。在此特別提醒各患者,若經 常遠遊,應及早求醫治理,因長時間乘坐飛機及汽車,可影響下肢血液流動,會加速病情惡化,增加引致淺表血栓性靜脈炎的機會。

Blisters



Blisters are caused by skin friction. Apply moleskin or an adhesive bandage over a blister, and leave it on until it falls off naturally in the bath or shower. Keep your feet dry and always wear socks as a cushion between your feet and shoes. If a blister breaks on its own, wash the area, apply an antiseptic, and cover with a sterile bandage. Sometimes blisters can be due to repetitive pressure from faulty footwear or underlying biomechanical problems.

Athlete’s foot/ Tinea Pedis





Athlete’s foot/ Tinea Pedis is a skin infection usually starting between the toes or on the bottom of the feet. It is caused by fungal infection that commonly attacks the feet, because the wearing of shoes and hosiery or poor foot hygiene can fosters fungus growth.

The signs of athlete’s foot are dry scaly skin, itching, inflammation, and sometimes blisters can be found. We can help prevent infection by washing your feet daily with soap and warm water; drying carefully, especially between the toes; and changing shoes and hose regularly to decrease moisture. Our podiatrist can advise you on some topical antifungal agent or advise you on proper hosiery or footwear / insole for prevention of the fungal growth.

「醫生,請不要把我的腿切除!」

他是一名患有腎衰竭及糖尿病的年長病人,數年前因下肢動脈血管閉塞,需要把左腿切除。靠著剩下的右腿,並在己截肢的左腿裝上義肢,加上無比的鬥志,仍能一拐一拐地走路,不用靠輪椅出入,自給自足。但慢慢地,他發覺健全的右腿越來越乏力,需要經常停下休息,才可繼續走路;近日,右腳腳趾甚至出現傷囗及皮膚變色,走路已相當困難。

經血管外科醫生檢查,原來無情的血管阻塞已蔓延至右腿,甚至部分組織己有壞死跡像,他不太覺得痛只因糖尿病己把腳部的神經系統變得麻木。

這可麻煩了, 醫生本想設辨法替他進行微創血管手術,把血液送到足部,以保存右腿,但擔心手術後壞死組織分解出來的毒素,會隨血液走遍全身,可引起嚴重並發症,甚至可致命!

保守點但較安全的做法,是棄車保帥,為右腿進行截肢,把小腿及壞死組織一並切除,以防壞肉影響身體其他機能;但他就會失去雙腿,以他的年紀,以後要再走路就不太可能了。

醫生跟病人及家屬商量,得到的答案是: 「醫生,請不要把我僅餘的腿切除!」

醫生內心爭扎著,若冒險替他進行通血管手術,讓他保留一線希望,成功的話,他的餘生便不用在輪椅上渡過;但要是不成功,他有機會賠上性命,家人到時可能會後悔,怪罪醫生;況且,醫院方面亦未必喜歡這樣冒險。還是簡單點,進行截肢,先保住性命再算?

Corns and calluses





Corns and calluses are protective layers of compacted, dead skin cells. They are caused by repeated friction and pressure from skin rubbing against bony areas or ill-fitting shoes. Corns and calluses can be found on the toes and soles of the feet. They are generally symptoms of underlying foot disorders. The friction and pressure can be relieved by padding or orthotics on the affected areas. Our podiatrist can remove the corns and calluses with sterialised instrument and assess the cause of the pressure lesions and advise you on proper footwear and inserts.
